Pay Rate:Up to $50/hr combined (taxable hourly + tax-free allowances for travel, housing, meals). Rates may vary and are subject to change. Additional Bonuses, Group Health Insurance benefits available on day 1, 401k and moreWe are seeking an experienced Registered Nurse (RN) to support Home Health and Hospice services in a clinic-based role. The Clinic RN will assist with coordinating patient care, reviewing and completing clinical documentation, communicating with patients and members of the interdisciplinary care team, and helping ensure timely and accurate completion of required documentation.The ideal candidate will have previous Home Health and/or Hospice experience and be comfortable working with OASIS documentation and charting requirements.Key ResponsibilitiesCoordinate and support patient care for Home Health and Hospice patients.Complete and/or review OASIS assessments and documentation as applicable to the role.Ensure patient charts and clinical documentation are accurate, complete, and submitted in a timely manner.Review patient information, physician orders, plans of care, medications, and changes in patient condition.Communicate with physicians, providers, field clinicians, patients, families, and other members of the interdisciplinary care team.Assist with admissions, recertifications, resumptions of care, transfers, and discharges as needed.Support development and updates of individualized patient plans of care.Monitor patient progress and communicate significant changes or concerns to the appropriate provider or care team.Provide patient and caregiver education regarding medications, treatments, disease management, safety, and care plans.Assist field staff with clinical questions and patient-care coordination.Follow agency policies and applicable Home Health, Hospice, Medicare, and regulatory requirements.Participate in quality improvement and documentation review activities as needed.Participate in an on-call rotation and assist with after-hours patient needs as required.Perform additional clinical and administrative nursing duties as assigned.QualificationsCurrent and unrestricted Registered Nurse (RN) license in Illinois.Previous Home Health and/or Hospice nursing experience preferred.OASIS charting and documentation experience preferred.Knowledge of Home Health and Hospice standards, clinical documentation, and care coordination.Strong assessment, critical-thinking, organizational, and communication skills.Ability to prioritize multiple patient and clinical needs in a fast-paced environment.Comfortable working independently while collaborating closely with an interdisciplinary team.Strong computer and electronic medical record (EMR) documentation skills
