Client Manager - Designated Coordinator
Job Description
WACOSA is currently seeking a compassionate and highly organized Client Manager/Designated Coordinator to join our team at our Sauk Centre location. The Client Manager/Designated Coordinator will be responsible for overseeing a caseload of clients with developmental, intellectual, or physical disabilities, ensuring they receive the services, support, and advocacy they need to thrive.
Pay: $22.08 - $23.84 per hour

Responsibilities
- Monitor and evaluate service delivery to ensure support plans (CSSPs) are implemented effectively.
- Track client progress toward identified outcomes and recommend plan modifications when needed.
- Provide instruction, coaching, and supervision to Direct Support Professionals (DSPs).
- Maintain effective communication with case managers, families, guardians, and other members of the support team.
- Ensure program compliance with all applicable 245D licensing requirements, policies, and procedures.
- Conduct site visits for persons served in individual placement settings.
- Maintain a visible presence within the program through role modeling, staff support, and quality oversight.
- Coordinate admissions, service changes, and discharges in collaboration with the Program Director.
- Develop, implement, and maintain person-centered service plans with measurable, outcome-based goals.
- Build positive relationships with clients while providing mentorship and guidance to staff.
- Develop positive behavior support strategies as needed in consultation with the Program Director and Mental & Behavioral Health Specialist.
Qualifications and Skills:
- Bachelors degree in human services, education, or health and 1-year direct care experience supporting persons with intellectual or developmental disabilities OR Associates degree in related field and 2 years direct care experience supporting persons with intellectual or developmental disabilities.
- Strong communication skills and confidence speaking with individuals and groups.
- The ability to understand and apply policies and procedures.
- Critical thinking and problem-solving abilities.
- Comfort using computers and learning new technology.
- Must be 18 years of age or older, possess a valid MN Driver’s License and insurable driving record.
- Ability to pass a DHS background check and drug screening.
