Graceful Touch Home Care LLC is seeking a dependable and professional PRN Registered Nurse to conduct in-home assessments and follow-up visits for our clients.

Responsibilities

• Conduct in-home health assessments and evaluate client needs.
  
  
• Monitor vital signs and changes in health status.
  
  
• Develop and update individualized care plans.
  
  
• Complete required documentation accurately and promptly.
  
  
• Communicate concerns or changes to the agency.
  
  
• Provide follow-up and supervisory visits as assigned.
  
  

Position Details

• PRN/as-needed schedule
  
  
• Home visits required
  
  
• Visits typically take approximately 30 minutes
  
  
• Minimal paperwork/documentation
  
  
• Independent travel within assigned service areas required
  
  

Qualifications

• Current South Carolina RN license
  
  
• CPR/BLS certification
  
  
• Valid driver's license and reliable transportation
  
  
• Strong assessment, communication, and time-management skills
  
  
• Home care experience preferred
