Registered Nurse (RN) – Home Care Case Manager (Part-Time)

Monday through Thursday, 9:00 AM–1:00 PM—No weekends, No holidays, and No on-call.

About the Role
This is a Skilled Case Management position—no clinical care required.
Schedule and perform Q60 day HHA Supervisory/Recertification visits
Schedule and perform Start of Care assessment visits
Ongoing Case Management for client and HHA oversight
Evaluate the client status and the skills of staff members
Complete timely and accurate clinical assessments within 3 days of the visit
Complete timely and accurate 485/ Plan of Care for patients within 3 days of the visit
Provides patient, family, or caregiver education
The role is Office based out of our Philadelphia Location with local travel, typically 3-4 home visits per day for brief assessments.

Monday through Thursday
9:00 AM - 1:00 PM
NO weekends
NO holidays
NO on-call

Qualifications
Active RN License in Pennsylvania
Valid Driver’s License and reliable vehicle
Personal laptop
PPD or QuantiFERON test
CPR Certification
Minimum 1 year of nursing experience (home health experience preferred)
