Absolut Care of Gasport is seeking an experienced MDS RN to join our team. The MDS RN plays a key role in ensuring accurate resident assessments, strong clinical outcomes, and compliance with all state and federal guidelines.
Responsibilities:
Complete, manage, and submit MDS assessments on time
Develop and update individualized care plans
Partner with nursing, therapy, and interdisciplinary teams
Support survey readiness and quality initiatives
Educate staff on documentation requirements
Requirements:
Current NYS RN license
Previous MDS/RAI experience required
Knowledge of PDPM, care planning, and clinical documentation
Strong organizational and communication skills
