As a Registered Nurse (Per Diem) on our Home Health team, you will play a vital role in delivering high-quality, patient-centered care to seniors in the comfort of their own homes. Our team is dedicated to providing exceptional care and support to our clients, and we are seeking a compassionate and skilled RN to join our per diem team.
Responsibilities:
• Provide skilled nursing services to clients in their homes, including assessing patient needs, developing and implementing individualized care plans, and educating patients and families on self-care techniques
• Collaborate with other healthcare professionals, including physicians, therapists, and home health aides, to ensure comprehensive care and support
• Document patient progress and communicate with the interdisciplinary team to optimize patient outcomes
• Participate in patient scheduling, care coordination, and other administrative tasks as needed
