Social Worker

We are seeking a full time Social Worker to join our team. The ideal candidate will have experience in a long term care or sub-acute setting and must have previous healthcare experience. WiIlling to train the right candidate.

Preference given to candidates with nursing experience (CNA, LPN, RN)

Duties include:

• Identifying individual social and emotional needs

• Developing and Maintaining individualized care plans

• Participating in the planning of the resident’s admission and discharge to the community

• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

• Assess residents on a regular basis

• Coordinate investigations for resident grievances

Requirements:

• Bilingual (Preferred)

• Experience with MDS 3.0

• Proficient in discharge planning and memory support

• Long term care/sub-acute experience required

• Familiarity with EMR and basic computer programs
