Job Summary

The Rehabilitation Specialist provides psychiatric rehabilitation services to individuals with serious mental illness and emotional disorders enrolled in the Psychiatric Rehabilitation Program (PRP). The Rehabilitation Specialist assists participants in developing, maintaining, and improving independent living, socialization, educational, vocational, and community integration skills necessary to achieve recovery goals identified in the Individual Rehabilitation Plan (IRP).
The Rehabilitation Specialist works collaboratively with consumers, families, treatment providers, community resources, and interdisciplinary team members to promote recovery, self-sufficiency, wellness, and successful community living.

Responsibilities and Duties

Direct Service Delivery

  • Provide individual and group psychiatric rehabilitation services in accordance with Maryland PRP regulations and COMAR requirements.
  • Assist participants in identifying strengths, needs, preferences, and recovery goals.
  • Implement interventions identified in the Individual Rehabilitation Plan (IRP).
  • Support participants in developing independent living skills, social skills, coping skills, and community integration skills.
  • Assist individuals with accessing community resources, educational opportunities, vocational services, transportation, housing, and healthcare services.
  • Facilitate skill-building groups and psychoeducational activities.
  • Promote recovery-oriented and person-centered practices.

Documentation

  • Complete rehabilitation assessments as assigned.
  • Participate in the development, implementation, and review of Individual Rehabilitation Plans (IRPs).
  • Maintain accurate, timely, and compliant documentation of services provided.
  • Complete progress notes within required timeframes.
  • Maintain participant records in accordance with agency policies and Maryland regulations.
  • Ensure documentation supports medical necessity and participant progress.

Care Coordination

  • Collaborate with psychiatrists, therapists, case managers, PRP staff, families, and community partners.
  • Participate in treatment team meetings and case conferences.
  • Assist with referrals and coordination of services.
  • Communicate significant participant concerns to supervisors and treatment team members.

Community-Based Services

  • Provide rehabilitation services in community settings when appropriate.
  • Assist participants in developing skills necessary for successful community functioning.
  • Support participants in practicing learned skills in real-life environments.
Compliance & Quality Assurance

  • Adhere to COMAR regulations governing Maryland PRP services.
  • Maintain participant confidentiality in accordance with HIPAA regulations.
  • Participate in audits, quality improvement activities, staff meetings, and training.
  • Follow agency policies regarding incident reporting and risk management.

Qualifications and Skills

  • Bachelor's degree in Psychology, Social Work, Counseling, Psychiatric Rehabilitation, Sociology, Human Services, or a related behavioral health field
  • One (1) year of experience providing mental health rehabilitation services.

Required Knowledge, Skills, and Abilities

  • Knowledge of recovery-oriented mental health practices.
  • Knowledge of serious mental illness and co-occurring disorders.
  • Ability to develop therapeutic relationships.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Strong organizational and documentation skills.
  • Ability to maintain professional boundaries and confidentiality.
  • Basic computer proficiency and electronic health record experience.

Preferred Qualifications

  • Certified Psychiatric Rehabilitation Practitioner (CPRP).
  • Experience in Maryland PRP services.
  • Bilingual abilities.
  • Experience working with diverse populations.

Physical Requirements

  • Ability to travel within the community.
  • Ability to sit, stand, bend, and walk for extended periods.
  • Ability to transport materials necessary for service delivery.

Work Schedule

Flexible schedule, including evenings and weekends as needed to meet participant needs.
