Director of Social Work
Located in Queens, NY
Salary: $85K to $95K Range; based on experience
Requirements:
- Must have prior nursing home / long-term care experience
- Previous experience as a Social Work Director
- Strong knowledge of discharge planning, care coordination, and resident advocacy
- Experience working with interdisciplinary teams
- Familiarity with state and federal regulations related to long-term care
- LMSW strongly preferred, but not required
Job Description:
- Plans and directs a comprehensive social services program which provides support services for residents and families.
- Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
- Participates in an interdisciplinary team approach to better serve the needs of the facility population.
- Oversee and manage the Social Services department
