Position Overview
The ideal candidate will have experience in a Long-Term Care (LTC) or Sub-Acute setting and a strong commitment to enhancing the social and emotional well-being of our residents.

Key Responsibilities:

• Conduct initial psychosocial assessments upon resident admission
• Coordinate discharge planning for short-term residents, including documentation and maintaining the facility discharge calendar
• Identify residents’ social and emotional needs and develop appropriate care plans
• Complete MDS-related assessments, including social work sections
• Schedule, document, and lead resident care conferences
• Participate in interdisciplinary care plan meetings to ensure comprehensive resident care

Requirements:

• Prior experience in Long-Term Care or Sub-Acute settings required
• Strong organizational, communication, and interpersonal skills
