Job Description
Clinical Manager:
Visiting Nurse & Hospice of Litchfield County is seeking a dedicated and experienced Clinical Manager to join our management team and oversee a group of highly skilled Registered Nurses (RNs) providing exceptional patient care in the home setting. The Clinical Manager will be responsible for ensuring that the team delivers high-quality care, while maintaining compliance with all regulatory standards, and promoting positive patient outcomes.
QUALIFICATIONS:
The Clinical Manager shall be a registered nurse with an active license to practice nursing in the State of Connecticut and shall have one of the following:
1. A master’s degree from a program approved by the National League for Nursing or the American Public Health Association with a minimum of one (1) year full-time clinical experience in a home health agency or related community health program which included care of the sick at home; or
2. A baccalaureate degree in nursing and minimum of three (3) years of full-time clinical experience in nursing, at least one (1) of which was in a home health agency or community health program which included care of the sick at home; or
3. A registered nurse who has been continuously employed in the position of Supervisor of Clinical Services in a home health agency in this state since January 1, 1979; or
4. A diploma in nursing and
a. A minimum of six (6) years of full-time or full-time equivalent clinical experience in nursing within the past ten (10) years, at least three (3) years of which were in a home health care agency or community health program which included care of the sick at home; and
b. Evidence of certification by the American Nurses Association as a community health nurse of completion of at least six (6) credits received within two years in community health nursing theory or six (6) credits in health care management form an accredited college or university program or school of nursing.
Other Qualifications:
· Knowledge of State and Federal Regulations governing provision of home health services and OASIS guidance and documentation
· Ability to supervise and coordinate the work of staff and effectively manage human resources
· Ability to facilitate cooperation and communication among team members
· Exhibits self-motivation, tact and professionalism
· Proficient in use of software applications
· Current Certification in Cardiopulmonary Resuscitation from the American Heart Association or American Red Cross
· Must possess a valid drivers license and personal vehicle
· Physical, mental and sensory abilities sufficient to meet performance criteria with or without assistive devices
