The W Assisted Living Management Company is seeking a compassionate and dedicated Lead Case Manager. We have our eye out for a candidate with a proven track record of successfully coordinating services for residents, while remaining committed to excellence in resident satisfaction. The chosen candidate will have the opportunity to work with a positive and supportive team of healthcare professionals, while helping guide and mentor fellow case managers.
Job Responsibilities:
Maintain firm adherence to the resident's "Bill of Rights," as they relate to the facility
Maintain meaningful communication with the resident's care providers
Lead assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
Coordinate and supervise clinical care for residents
Serve as a point of guidance for other case managers, helping ensure consistency in process and documentation
Clear and concise documentation in accordance with facility requirements
Assist in training and onboarding new case management staff
Qualifications/Requirements:
2+ years' experience as a Case Manager, ideally within Long Term Care
Previous case management and Compuware experience a plus
Excellent phone skills and ability to work with families/other care providers
Strong organizational skills, passion and compassion for the population
Ability to lead and work cooperatively with a multidisciplinary team
Bachelor's Degree preferred but not required
