Home Health Aide HHA

We are seeking a compassionate and detail-oriented Home Health Aide (HHA) to provide personalized, one-on-one care to patients in their homes. In this role, you will assist with daily living activities, ensure patient comfort and safety, and build meaningful relationships while following individualized care plans under RN supervision. Strong communication skills and a commitment to patient well-being are essential, with opportunities for continued training and career advancement.

Responsibilities

• Assist patients with activities of daily living (bathing, grooming, dressing, toileting, mobility)
• Prepare meals, perform light housekeeping, and offer companionship
• Monitor and report changes in patient condition, behavior, or environment to supervising nurse
• Adhere to physician orders and individualized care plans under RN supervision
• Accurately document care and complete Electronic Visit Verification (EVV) for each visit
• Maintain a safe, clean, and supportive environment while respecting patient privacy and dignity
• Communicate effectively with patients and healthcare team
• Ensure compliance with safety protocols and time management during visits

Required Qualifications

• Home Health Aide License or CNA License
• High School Diploma or GED
• CPR certification
• Experience with patient care and ADL assistance
• Proficiency with Electronic Visit Verification (EVV) systems
• Basic housekeeping skills
• Strong communication, dependability, and time management skills
• Awareness of safety and ability to maintain a safe environment

Preferred Qualifications

• One or more years of professional experience in home health care
