We are looking for a Care Coordinator to support care management operations and help ensure members receive timely, well-organized services in Syracuse, New York. This contract-to-permanent opportunity is ideal for someone who is comfortable balancing administrative coordination, data accuracy, and ongoing communication with patients, providers, and internal teams. In this role, you will contribute to authorization workflows, service scheduling, and reporting while helping the team maintain efficient and responsive support for member needs.
Responsibilities:
• Maintain accurate authorization records in electronic systems and monitor upcoming expirations to support uninterrupted services.
• Coordinate approved care services with external agencies and assist the team in arranging appointments and related support activities.
• Communicate regularly with members to provide updates, answer routine questions, and help address service-related needs.
• Partner with clinical staff and supervisors to review workflows, share observations, and support process enhancements.
• Prepare, update, and manage operational reports as requested to help track team activity and service delivery.
• Provide day-to-day administrative support through document preparation, filing, copying, message handling, and other office tasks.
• Handle referral and authorization activities with close attention to detail, timelines, and established procedures.
• Complete additional duties assigned by leadership to support departmental priorities and member care coordination.• High school diploma required; an associate degree in a related field is preferred.
• At least 2 years of experience in healthcare administration or a related healthcare setting, or completion of training as a Medical Assistant.
• Strong data entry and computer skills, with confidence using Microsoft Office and other business applications.
• Demonstrated ability to analyze information, recognize issues, and contribute to practical solutions.
• Experience managing multiple priorities independently while also collaborating effectively with a team.
• Strong verbal and written communication skills, including clear and attentive interaction with members and internal staff.
• Ability to handle confidential and sensitive information with sound judgment and discretion.
• Background in scheduling, customer service, and general administrative support is highly valued.
