CHW Care Coordinator - Cold calling - Remote Louisville

BCforward is currently seeking a highly motivated "CWH Care Coordinator II" for an opportunity in Remote – Must reside in Kentucky

Position Title: Care Coordinator II

Location: Remote – Must reside in Kentucky

Duration: 6 months with possibility to extend or convert to FTE

Shift Timing: 9:00 AM – 5:30 PM EST/CST.

Pay Rate: $23/hr

Note: temp workers will be asked to go in person to staff meetings once a quarter at Client hub - Louisville or Lexington, not a requirement of the job right now but if hired full-time they will be expected to go to community events as needed. Will be doing digital eye exams (if licensed), assisting with coverage/connections, etc.

Position Summary

This role supports member outreach, care coordination, and preventive health efforts for WellCare of Kentucky. The position focuses on outbound member outreach, care coordination support, and helping close preventive care gaps such as well-child visits, postpartum care, A1C testing, and breast cancer screenings.

Responsibilities

• Make and receive member outreach calls.
• Educate members on healthcare services and benefits.
• Assist members with appointment scheduling.
• Document outreach activities and member interactions.
• Coordinate follow-up activities.
• Support outreach initiatives related to:
• • Well-child visits
  • Postpartum care
  • A1C testing
  • Breast cancer screenings
• Assist with coverage and community resource connections.
• Support care coordination activities for SDOH resource needs.

Required Qualifications

• High School Diploma or GED.
• 1–2 years of related experience.
• Customer service experience.
• Member outreach experience.
• Provider scheduling experience.
• Community resource knowledge.
• Medical or clinical knowledge.
• Remote work experience.

Preferred Qualifications

• Community Health Worker (CHW) certification.
