Job Description
The Registered Nurse (RN) is responsible for assessing, planning, implementing, and evaluating nursing services provided to participants enrolled in the Adult Day Health (ADH) program. The RN ensures participant health and safety, coordinates care with physicians and multidisciplinary team members, oversees medication administration, develops individualized care plans, and maintains compliance with Massachusetts Department of Public Health (DPH) Adult Day Health regulations. Responsibilities align with participant assessments, care planning, physician orders, and medication management functions identified in Massachusetts ADH compliance materials.
Clinical Care
- Perform initial and ongoing nursing assessments of participants.
- Monitor participant health status and identify changes in condition.
- Develop, implement, and update individualized care plans.
- Administer medications and treatments according to physician orders.
- Respond to participant medical emergencies and provide appropriate interventions.
- Maintain accurate clinical documentation and nursing notes.
Care Coordination
- Communicate with physicians, families, caregivers, and community providers.
- Obtain and process physician orders.
- Coordinate participant referrals and follow-up care.
- Participate in interdisciplinary care team meetings.
Health Monitoring & Education
- Monitor vital signs, blood glucose levels, and other clinical indicators as needed.
- Provide participant and caregiver education regarding chronic disease management, medications, nutrition, and preventative health.
- Promote participant wellness and independence.
Regulatory Compliance
- Ensure nursing services comply with Massachusetts DPH Adult Day Health requirements.
- Maintain participant records, assessments, physician orders, and care plans.
- Monitor infection control practices and participant safety measures.
- Participate in quality improvement and audit activities.
Staff Support
- Provide nursing guidance to program aides and other staff.
- Participate in staff training and competency reviews.
- Assist with development of health and safety procedures.
Qualifications
- Current Massachusetts Registered Nurse (RN) license in good standing.
- Graduate of an accredited nursing program.
- Minimum 1-2 years of clinical nursing experience preferred.
- Experience with geriatrics, adult day health, long-term care, home care, or community health preferred.
- Current CPR/BLS certification.
- Strong assessment, communication, and documentation skills.
- Ability to work effectively with elderly and disabled adults.
