The Phone Nurse role for our clinic plays a crucial role in delivering exceptional over the phone patient care by conducting thorough assessments, coordinating care, and providing education and support. You have the clinical knowledge to differentiate between a simple question and a medical emergency. We treat our patients like family, and that includes taking the time to hear what they REALLY need.


Working within a medium-sized team, the Phone Nurse collaborates closely with the Clinical Supervisor and other healthcare professionals to ensure seamless communication and effective follow-ups. This position operates during normal business hours with no travel required and utilizes athenaOne EMR for documentation. We have a patient population that runs from 0-103, and calls that go from a simple question about a new medication to complex chronic care issues.

Responsibilities

  • Conduct telephone triage to assess patient symptoms and provide guidance
  • Perform patient assessments over the phone and document accurately
  • Coordinate care with healthcare providers and schedule same day appointments when necessary
  • Educate patients on health management and follow care plans
  • Document all patient interactions and triage decisions using athenaOne EMR
  • Communicate effectively with patients and the healthcare team
  • Make follow-up calls to monitor patient progress and ensure continuity of care

Preferred Qualifications

  • 1+ years of clinical nursing experience
  • Licensed Vocational Nurse (LVN) license
  • Skilled in patient assessment and telephone triage
  • Experience with Electronic Medical Records (EMR), especially athenaOne
  • Strong communication and critical thinking skills
  • Proficient in patient education and time management
