Responsibilities:
Conduct thorough patient assessments, including minimum data set (MDS) assessments, to identify patient care needs and develop individualized care plans
Collaborate with the interdisciplinary team to implement care plans, providing education and guidance to patients, families, and caregivers
Monitor and report changes in patient condition, and make recommendations for revisions to care plans as needed
Participate in quality improvement initiatives, including data collection and analysis, to ensure optimal patient outcomes
Maintain accurate and timely documentation of patient assessments, care plans, and progress
About the Role:
