The Home Health Caregiver provides essential support and assistance to individuals in their homes, ensuring comfort, safety, and well-being. Working primarily in daytime shifts, the caregiver operates independently while reporting to a Registered Nurse, and utilizes electronic health records to maintain accurate documentation.
Responsibilities
- Provide personal care and hygiene assistance
- Support patient mobility and transfers
- Assist with daily living activities
- Monitor health status and report changes
- Prepare meals according to dietary needs
- Offer companionship and emotional support
- Perform light household duties
- Document care and observations in electronic health records
Required Qualifications
- Minimum 1 year of experience in home health care
Preferred Qualifications
- High school diploma or equivalent
- CPR certification
- Home Health Aide certification
- State Tested Nursing Assistant certification
- Strong patient care skills
- Effective communication abilities
