Primary Care Provider
Alliance Family Health Center, Inc.
Basic Function
Alliance Family Health Center (AFHC) is a Federally Qualified Health Center Look-Alike looking to fill a Primary Care Provider position. The Provider is responsible for providing comprehensive healthcare across the lifespan in both the primary care and urgent care modality. The Primary Care Provider will focus on health promotion, disease prevention, health maintenance, health restoration, and health education. They will assess issues or conditions including acute and stable chronic health problems. A Primary Care Provider’s daily responsibility includes providing primary and urgent care services with the collaboration of integrated behavioral health and women’s health professionals.
Job Duties:
- Provides comprehensive medical services to patients presenting for care at AFHC, on a continuing
basis; examines patients, using medical instruments and equipment.
- Using evidence-based guidelines, obtains appropriate patient history, performs physical evaluation, orders appropriate diagnostic tests and follows-up with interpretation, formulates assessment and initiates treatment plan with appropriate follow up appointments.
- Reviews incoming reports (e.g. lab, x-ray, EKG) signs, dates and follows up with patients in a timely manner.
- Administers or prescribes treatments and medications and instructs patients on proper utilization.
- Provide or directly oversee services and procedures appropriate in the outpatient setting.
- Inoculates and vaccinates patients to immunize patients from communicable diseases.
- Participates as an active member of the medical staff and participates in daily care team huddles to ensure prioritization of patient needs.
- Consults and refers with/to appropriate Specialty Physicians.
- Responsible for timely and accurate completion of medical records, including assessment, treatment plan, progress notes, outcome measures, and billing entries.
- Adheres to UDS productivity and quality standards and meets minimum requirements.
- Monitor patient progress and revise treatment plans as necessary.
- Observe all infection control procedures.
- Participates in quality and performance monitoring through review of patient outcomes.
- Perform other duties as assigned.
Required Education and Experience:
- FNP or PA-C, MD or DO licensure
- Minimum of one year clinical experience in a healthcare setting preferred
- Valid DEA required.
- Current BLS Certification

Age Appropriate Care:

Machines, tools, and equipment
1. Must be knowledgeable in equipment and supplies used in medical office.
2. Use of computer software systems including word, excel, medical records systems.
Physical Requirements:
1. Normal space, ventilation, and noise levels
2. Must be able to get up and down out of chair frequently throughout the shift
3. Must be able to lift 40 lbs.
4. Ability to stand on feet for several hours
5. Must possess sight and hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met
6. Must be able to verbalize well and possess manual dexterity type and/or write legibly
