Duties

  • Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using proper patient transfer methods and equipment such as Hoyer lifts.
  • Monitor and document vital signs accurately, including blood pressure, pulse, respiration, and temperature, adhering to medical terminology standards.
  • Administer medications and perform catheter care following home healthcare regulations and medication administration protocols.
  • Observe patients closely for changes in condition or behavior, maintaining detailed medical records within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while ensuring compliance with HIPAA confidentiality standards.
  • Provide specialized dementia care, Alzheimer’s care, memory care, hospice support, or end-of-life assistance with sensitivity and professionalism.
  • Support patients with mobility challenges through patient transfer methods and assist with light housekeeping tasks such as cleaning, laundry, cooking nutritious meals, and maintaining a safe environment.
  • Communicate effectively with clients and their families to ensure clarity about care plans and respond promptly to their needs or concerns.

Qualifications

  • Proven experience working with individuals with disabilities, developmental disabilities, geriatrics, or in assisted living environments.
  • Knowledge of home health care assistance practices including patient personal care assistance and home & community care experience.
  • Familiarity with medical terminology, medical documentation procedures, and home health regulations.
  • Certification in First Aid and Basic Life Support (BLS), including CPR training.
  • Ability to perform heavy lifting safely when necessary for patient transfers or mobility support.
  • Experience working with EMR/EHR systems for accurate recordkeeping; prior home health agency experience is preferred.
  • Strong communication skills to effectively interact with patients, families, and healthcare professionals while maintaining patient confidentiality under HIPAA guidelines.
  • Valid training or experience in hospice care or working with geriatric patients is highly desirable.

  • Friday, Saturday, Sunday 8am-6pm
