Summary:
The Licensed Vocational Nurse (LVN)/ Licensed Psychiatric Technician (LPT) is responsible for providing medication supportive services under the direction of the Medical Director to the CJSP population with mental health challenges. The LVN/LPT will provide direct patient care through the use of standardized procedures and protocols as approved by the Medical Director; follow departmental policies and procedures; exercise initiative, originality and judgment in applying established principles and practices to new and unusual problems; serve as a resource for staff and participate in educational sessions for staff. General supervision is provided in the form of chart review and consultation and collaboration with the Medical Director.



Essential Duties and Responsibilities:
include the following. Other duties may be assigned;

• Provide direct care and support to clients with mental health disorders, including administering medications by injections/oral, monitoring vital signs, and assisting with daily activities.

• Conducts assessments, service plan development, case management, crisis intervention;

• Implement and follow individualized treatment plans as directed by the healthcare team.

• Observe and report changes in clients' behavior, physical condition, and response to treatment.

• Conduct crisis intervention and de-escalation techniques when necessary.

• Assist in the development and modification of care plans in collaboration with the treatment team.

• Maintain accurate and timely documentation of client care, treatments, and progress notes’ responsible for maintaining assigned productivity levels each month.

• Participate in team meetings, case conferences, and ongoing training sessions.

• Educate clients and their families about mental health conditions, treatment plans, and available resources.

• Ensure compliance with all relevant laws, regulations, and organizational policies. Serves as a resource for staff and provides consultation;

• Assists Medical Providers and coordinates with the clerical and treatment team on client care;

• Attends medication support meetings and serves on committees as needed;


Supervisory Responsibilities:
This position has no supervisory responsibilities.

Education and/or Experience: Minimum qualifications are as follows:
• Possession of a current, valid California License Vocational Nurse (LVN) license; OR Possession of a current valid California Psychiatric Technician (LPT) license.
• Minimum of two (2) years full-time experience in an outpatient mental health setting or acute care facility;

• Experience working with older adults (preferred)

• Active CPR Certified
