About the Role:
Provide personalized care and support to clients in their homes, focusing on their unique needs and preferences
Utilize your expertise in Hoyer lifts to assist with safe transfers, ensuring the comfort and dignity of our clients.
Collaborate with our interdisciplinary team to develop and implement personalized care plans that promote independence, comfort, and overall well-being
Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination
Maintain accurate and timely records of client care and services, adhering to all regulatory and company standards
Responsibilities:
Provide routine personal care, including bathing, grooming, and dressing
Assist with activities of daily living (ADLs), such as toileting, transferring, and
Monitor and report changes in client condition or behavior to Care Coordinators and healthcare providers
Participate in ongoing education and training to stay current with industry best practices and regulatory requirements
