As a Certified Nursing Assistant (CNA) at Cliveden Nursing and Rehabilitation Center, you will play a vital role in delivering exceptional care to our patients. Our subacute care facility is located in Philadelphia, within walking distance of Germantown, and provides short-term post-acute rehabilitation as well as long-term skilled nursing care.
About the Role:
Work with a dynamic and innovative team to provide personalized care to patients from several area hospitals, including Temple Hospital, Roxborough Hospital, Albert Einstein Medical, and Chestnut Hill Hospital.
Provide direct patient care, including assisting with activities of daily living, vital signs, and mobility.
Collaborate with our interdisciplinary team to develop and implement individualized care plans.
Maintain accurate and timely documentation of patient care.
Support a culture of excellence, infection control, and quality improvement initiatives.
