August Healthcare at Iliff is seeking a motivated and patient-focused Physical Therapist (PT) to join our rehabilitation team. In this role, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services that promote mobility, independence, and quality of life. You will collaborate with an interdisciplinary team to ensure high-quality, patient-centered care while maintaining compliance with regulatory and reimbursement standards.

ResponsibilitiesEvaluate residents and develop individualized treatment plans based on physician orders and clinical findings
Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and functional independence
Reassess resident progress and modify treatment plans as appropriate
Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, and discharge summaries
Maintain productivity standards established by the department
Communicate resident progress, concerns, and recommendations with physicians, nursing staff, families, and the interdisciplinary care team
Supervise and collaborate with Physical Therapist Assistants (PTAs) and rehabilitation support staff as appropriate
Participate in interdisciplinary team meetings, care conferences, and utilization reviews
Educate residents, families, and caregivers on treatment plans, home exercise programs, and safety recommendations
Assist with maintaining the cleanliness and organization of therapy areas and equipment
Report equipment issues to the appropriate personnel
Participate in facility performance improvement initiatives
Travel or float between facilities as assigned based on resident needs
Maintain compliance with Medicare, reimbursement, and regulatory guidelines
Graduate of an accredited Physical Therapy program
Current state licensure or registration as a Physical Therapist
Knowledge of Medicare and reimbursement regulations
Strong clinical, communication, and documentation skills
Ability to work collaboratively within an interdisciplinary team
Dependable, organized, and patient-focused
Ability to stand, walk, bend, lift, and assist residents throughout the shift
Ability to lift up to 50 pounds
Exposure to clinical environments and patient care settings
Ability to manage the physical and emotional demands of patient care
