We are looking for a reliable LVN/LPN (Licensed Practical Nurse/Licensed Vocational) to administer direct patient care, administer approved medications, perform treatments, make accurate nursing observation, and chart on the patients' medical record in a professional and considerate manner. Duties are performed under the direction of the Case Manager and/or Patient Care Manager. LVN/LPN professionals must be first and foremost qualified and compassionate. The ideal candidate preference will have at least one year experience as a nurse in a health care setting. Home health experience preferred. We offer competitive pay and flexible schedule.

Educational Requirements:
- Graduate of an approved program of vocational/practical nursing
- Current Licensed Vocational/Practical Nurse (LVN/LPN) from the Texas Vocational Nurse Examiners
- Current CPR
- BLS certification preferred.
- Ability to perform patient care and demonstrate the use of standard equipment per competency skills inventory.
- Ability to accept assigned duties and work in a cooperative manner.
- Excellent verbal and written communication/documentation skills
- Professional in appearance and practices good personal hygiene
- Able to see/hear/speak sufficiently to participate in written/verbal communication.
- Able to deal effectively with stress.
- Able to use fine motor skills for patient assessment.
- Able to stand, sit, bend, stoop, ambulate uneven surfaces, and lift up to 50 pounds.
- Able to operate a motor vehicle and travel to prospective patient’s residences.
- Assists in assessing patient and family deeds through the recognition and reporting of signs and symptoms and nursing observation to the assigned Case Manager
- assists in assessing the health status of assigned patients.
- complies with the established nursing assessment guidelines/policies.
- Utilizes the written plan of care for implementing nursing interventions necessary to accomplish the established goals.
- Complies with general performance standards of licensed vocational nurse. Demonstrates flexibility in response to unexpected changes in work volume, emergencies, staffing, or scheduling changes.
- Must have a reliable means of transportation, proof of financial responsibility (car insurance and a valid state driver’s license)
