LPN Wound Care Job Description

An LPN (Licensed Practical Nurse) in Wound Care provides direct, specialized care to patients with acute or chronic wounds, working under the supervision of RNs or physicians. This role focuses on wound assessment, treatment, and patient education, with the goal of promoting healing, preventing infection, and improving quality of life


Key Responsibilities
  • Assessment & Evaluation: Examine wounds to determine type, severity, and appropriate treatment
  • Treatment & Dressing: Clean and dress wounds using sterile techniques; perform debridement as required
  • Care Planning: Assist in developing and implementing individualized wound care plans
  • Monitoring & Documentation: Track healing progress, update treatment plans, and maintain accurate records
  • Patient Education: Teach patients and families about wound care best practices, infection prevention, and self-care
  • Collaboration: Work with RNs, physicians, and other healthcare professionals to optimize patient outcomes
  • Supply Management: Ensure availability of wound care supplies and maintain inventory
  • Emotional Support: Provide compassionate care and support to patients and families
  • Monitor wound healing and make recommendations to RNs
  • Clean and bandage injured areas
  • Participate in quality improvement initiatives related to wound care
  • Educate patients on wound care and infection prevention
Requirements:

  • Education: LPN/LVN license
  • Clinical Skills: Proficiency in sterile wound care techniques, debridement, and infection control.
  • Communication: Ability to explain care instructions clearly to patients and families.
  • Attention to Detail: Accurate documentation and monitoring of wound changes.
  • CPR Certification
