CareDiem Home Health is seeking compassionate and reliable Home Health Aides to provide services to patients in Rockford. Our mission is to help clients remain safely at home while receiving top-quality, personalized care.

What You’ll Do:

• Provides Personal Care

• Perform simple procedures as an extension of therapeutic services

• Ambulation and exercise of the patient

• Household services essential to health care at home

• Assistance with medications that are ordinarily self-administered

• Reporting changes in the patient’s condition and needs to the registered nurse

• Completion of appropriate clinical records to record care given including observations and treatments.

What We’re Looking For:

• Training as a Home Health Aide and completed competency evaluation is required

• HHA must currently be on the Illinois State Registry

• 1+ year experience (home health preferred)

• CPR certification (BLS)

• Reliable transportation, valid driver’s license, and auto insurance

• Strong judgement, communication, and independence
