Responsibilities:
Provide personalized care and assistance to our residents, including bathing, dressing, grooming, and toileting
Assist with daily living activities such as feeding, transferring, and ambulating residents
Monitor and report changes in resident's condition to the charge nurse or nurse manager
Perform vital signs and heights and weights measurements as needed
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Maintain accurate and detailed records of resident care and services provided
