Responsibilities:
- Evaluate patients' physical conditions and develop individualized treatment plans to restore function and improve mobility
- Provide skilled physical therapy services in patients' homes in accordance with physician orders and plan of care
- Monitor and document patient progress, adjusting treatment plans as needed to ensure optimal outcomes
- Educate patients and family members on therapeutic exercises, safety techniques, and home programs
- Collaborate with interdisciplinary care teams including physicians, nurses, and other healthcare professionals
- Maintain accurate and timely clinical documentation in compliance with regulatory and agency standards
- Participate in discharge planning and coordinate continuity of care for patients transitioning between levels of care
- Valid Florida Physical Therapist (PT) license in good standing
- Graduate of an accredited Physical Therapy program (Master's or Doctorate preferred)
- Home health or outpatient experience preferred; new graduates encouraged to apply
- Strong knowledge of physical rehabilitation, therapeutic exercise, and manual therapy techniques
- Excellent communication, time management, and organizational skills
- Reliable transportation and valid driver's license for home visit travel throughout Port Charlotte, FL
- CPR/BLS certification required or ability to obtain upon hire
