Kairos Home Care Services is a growing Nurse Registry serving clients and families throughout Central Florida. We are seeking a Service Coordinator to take ownership of the day-to-day coordination of assigned client cases. This is a hands-on position responsible for case coordination, caregiver staffing, scheduling, authorization monitoring, service follow-up, client/family communication, and documentation. The ideal candidate is organized, proactive, dependable, detail-oriented, and comfortable communicating with clients, families, caregivers, and healthcare team members.
KEY RESPONSIBILITIES
Manage assigned client cases and take ownership of day-to-day case coordination.
Serve as a primary point of contact for assigned clients and families.
Communicate professionally with clients, families, caregivers, referral sources, and internal team members.
Review and monitor current service authorizations, including service codes, approved hours, effective dates, and expiration dates.
Verify that scheduled services align with current authorization requirements before creating or changing schedules.
Identify and follow up on authorization issues that may affect staffing or service delivery.
Proactively identify staffing needs and assist with finding available caregivers for assigned cases.
Work with the Staffing & Administrative Coordinator to identify potential caregiver matches and fill open or uncovered shifts.
Confirm caregiver availability and assist with caregiver/client introductions when appropriate.
Create, maintain, and update assigned client schedules in AxisCare.
Coordinate approved schedule changes and communicate changes to caregivers and families.
Monitor uncovered, missed, or disrupted services and follow up promptly.
Monitor service delivery and address client, family, caregiver, or scheduling concerns.
Maintain accurate and timely case documentation, staffing notes, schedule updates, and client communications.
Maintain confidentiality and ensure client and case information remains accurate and current.
Communicate unresolved staffing, authorization, service, scheduling, client, or compliance concerns to the Lead Service Coordinator.
Collaborate with the Staffing & Administrative Coordinator, Intake/Authorization team, HR/Caregiver Credentialing team, and management as needed.
Assist with internal reviews, audits, and other case management or administrative projects as assigned.

Experience in one or more of the following is preferred:
Home care
Nurse registry
Service coordination
Healthcare case management
Healthcare staffing
Caregiver scheduling
Medicaid services
Authorization management
Healthcare administration
Healthcare or home care experience is preferred but not required for the right candidate.
