About the Role
TruCare Management is seeking motivated and detail-oriented Certified Nursing Assistants (CNAs) to join our growing Chronic Care Management (CCM) team. This is a non-clinical, in-office position focused on coordinating care for patients with chronic conditions.
You will play a key role in improving patient outcomes by maintaining consistent communication, tracking health data, and supporting providers with documentation and care planning.
Key Responsibilities
Conduct monthly patient outreach via phone
Document all patient interactions in the EMR system
Track and manage patient care plans
Coordinate with providers, nurses, and facility staff
Identify changes in patient condition and escalate when appropriate
Ensure compliance with CMS guidelines for CCM services
Assist with care gap identification and follow-ups
Qualifications
Active CNA certification (required)
Strong communication and interpersonal skills
Comfortable working on phones for extended periods
Basic computer skills (EMR experience preferred)
Highly organized and detail-oriented
Ability to work in a fast-paced, team environment
