Job Summary:

Responsibilities:

• Provide direct patient care, including vital sign monitoring, wound dressing, and assist with activities of daily living (ADLs) such as bathing, dressing, and toileting

• Assist with transferring residents, mobility, and range of motion exercises

• Report any changes in residents' conditions, concerns, or needs to the nurse and other care team members

• Assist with medication administration and treatments under the supervision of licensed nurses

• Maintain accurate records and documentation of resident care and services

• Participate in resident care conferences and report to the care team

• Foster a positive and supportive environment for residents, families, and colleagues
