Job Summary:

About the Role:
As a Certified Nursing Assistant Home Health Aid, you will:
Provide personalized care services to clients in the comfort of their own homes, including assistance with activities of daily living (ADLs), personal care, and homemaking tasks
Monitor and report changes in clients' conditions to registered nurses and other healthcare professionals
Collaborate with healthcare teams to develop and implement individualized care plans
Maintain accurate and up-to-date records of client care services and progress
Provide emotional support and companionship to clients and their families
Responsibilities:
Deliver high-quality, patient-centered care to clients
Demonstrate strong communication and interpersonal skills
Work effectively as part of a multidisciplinary care team
Maintain confidentiality and uphold professional boundaries
Participate in ongoing training and education to stay current with industry best practices
Requirements:
Current certification as a Certified Nursing Assistant (CNA) in the state of Louisiana
High school diploma or equivalent
At least 3 years of experience in home health care or a related field
Ability to travel to client homes within the Baton Rouge, LA area
