Homeside Rehab is seeking compassionate and motivated Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) to provide high-quality, individualized therapy services to patients in their homes throughout Fort Lauderdale, Coral Gables and Port St. Lucie, Florida.

Responsibilities

• Perform comprehensive occupational therapy evaluations.

• Develop individualized treatment plans based on patient needs and goals.

• Provide evidence-based occupational therapy interventions to improve functional independence and quality of life.

• Educate patients and caregivers on treatment plans and home exercise programs.

• Maintain accurate and timely electronic documentation.

• Collaborate with physicians, caregivers, and interdisciplinary team members to ensure coordinated care.

• Monitor patient progress and modify treatment plans as appropriate.

Qualifications

• Active Florida Occupational Therapy License (OT) or Certified Occupational Therapy Assistant (COTA) License (Required)

• Minimum one year of experience in Home Health, Outpatient, Skilled Nursing Facility (SNF), or related clinical setting preferred

• Strong clinical reasoning and communication skills

• Comfortable working independently in patients' homes

• Basic computer skills for electronic documentation

• Valid Florida driver's license

• Reliable transportation
