Job Summary:
We are seeking a highly motivated and skilled Physical Therapist (PT) to join our team on a Per Diem basis. As a Physical Therapist (PT) at Desert Peak Care Center, you will be responsible for delivering high-quality patient care, assessing patients' conditions, developing and implementing treatment plans, and collaborating with other healthcare professionals to achieve optimal patient outcomes.
Responsibilities:
Assess patients' conditions and develop individualized treatment plans to achieve optimal patient outcomes
Conduct physical therapy sessions, including exercises, stretches, and other modalities to improve patients' strength, flexibility, and mobility
Monitor patients' progress, adjust treatment plans as needed, and communicate with patients and families to ensure seamless care
Collaborate with other healthcare professionals, including physicians, occupational therapists, and speech-language pathologists, to provide comprehensive patient care
Maintain accurate and detailed records of patient care, treatment plans, and progress
Stay up-to-date with best practices in physical therapy and adhere to professional standards and guidelines
Requirements:
Bachelor's or Master's degree in Physical Therapy (PT) from an accredited program
Current Physical Therapy license in the state of Arizona
Excellent communication, interpersonal, and problem-solving skills
Ability to work effectively in a fast-paced environment and adapt to changing priorities
