Responsibilities:
Provide direct patient care, including but not limited to, bathing, dressing, grooming, and toileting
Assist with medication administration, as certified and trained
Assess and report changes in patient condition to the nurse or physician
Maintain accurate and detailed records of patient care and services provided
Work closely with the interdisciplinary care team to develop and implement personalized care plans
Communicate effectively with patients, families, and healthcare professionals to ensure seamless coordination of care
Participate in ongoing training and professional development to stay current with industry best practices and standards
