Everbloom Memory Care is seeking an experienced, highly organized and proactive LPN Lead Clinician tojoin our leadership team. We currently serve approximately 22 residents and are growing toward 30residents. We have a strong full-time Administrator overseeing facility operations, and we are looking for aclinical leader who can take ownership of the day-to-day clinical department. This is a hands-on leadershipposition for someone who takes initiative, meets deadlines, develops strong med techs and followsresponsibilities through to completion without needing constant reminders or direction.Clinical Leadership Oversee day-to-day clinical needs of our residents. Monitor changes in condition and ensure appropriate and timely follow-up. Coordinate with physicians, pharmacies, hospice, home health and other healthcare providers. Ensure clinical documentation, orders and follow-ups are completed accurately and on time. Maintain organized, survey-ready clinical systems. Identify problems early and take initiative to resolve them. Work closely with the Administrator as part of the leadership team. Coordinate with RN support for responsibilities requiring RN involvement.Resident Assessments & AdmissionsThe Lead Clinician is expected to take active ownership of the clinical admission process rather thanroutinely relying on ownership to complete it. Complete prospective resident assessments within LPN scope and facility processes. Review medical records, medications, diagnoses, behaviors and care needs. Help determine whether the community can safely meet a prospective resident's needs. Communicate with families, hospitals, discharge planners and placement agencies. Respond promptly to assessment and admission opportunities. Coordinate clinical preparation for new admissions. Ensure staff understand a new resident's needs prior to or immediately following move-in. Follow new residents closely after admission and address concerns promptly.Med Tech Leadership & Training Train, mentor and develop medication technicians. Ensure med-tech training and competencies remain current. Provide ongoing education and retraining as necessary. Conduct medication, MAR and documentation audits. Identify medication errors and implement appropriate follow-up and education. Ensure med techs understand change-of-condition procedures and when clinical escalation isnecessary. Maintain organized systems for tracking training and competencies.Staff Education & Accountability Identify clinical training needs. Assist with orientation and education of new staff. Reinforce appropriate documentation and resident-care procedures. Provide coaching when standards are not being met. Help create a culture of teamwork, accountability and excellent resident care.On-Call & Scheduling Support Participate in clinical on-call responsibilities. Provide clinical guidance to staff when appropriate. Assist the Administrator with clinical staffing and scheduling needs. Help develop staff so they can appropriately manage routine situations and recognize when escalation isnecessary.Admissions, Marketing & Growth Partner with the Administrator to support census growth. Build positive relationships with families and referral partners. Participate in tours when clinical expertise is beneficial. Respond promptly and professionally to hospitals, discharge planners, placement agencies, hospice andother referral sources. Participate in occasional community outreach and networking opportunities. Represent Everbloom professionally and help families feel confident in the care their loved one willreceive.Qualifications Current Oregon LPN license in good standing. Memory Care, Assisted Living, RCF or long-term-care experience strongly preferred. Clinical leadership experience preferred. Experience training med techs/caregivers preferred. Strong assessment and medication-management skills. Excellent organizational and time-management skills. Strong communication and leadership abilities. Dependable, accountable and solution-oriented. Must be comfortable taking initiative and working independently.Compensation$80,000-$95,000/year + $500 per successful admissionPLUS $500 bonus for each successful resident admission.Who Will Be Successful Here?We are looking for more than a good clinician.
