As a Physical Therapist Per Diem, you will play a vital role in our multidisciplinary team, providing exceptional patient care and rehabilitation services to our residents. Responsibilities:
Provide physical therapy services to residents, including assessment, treatment planning, and implementation of plans
Develop and implement individualized treatment plans tailored to each resident's needs
Collaborate with other healthcare professionals to ensure effective communication and coordination of care
Conduct evaluations and assess residents' progress, making necessary adjustments to treatment plans
Maintain accurate and detailed records of patient care and treatment
Participate in staff meetings and educational in-services to stay current with industry best practices
Requirements:
Graduate degree in Physical Therapy from an accredited program
Current licensure to practice as a Physical Therapist in the state of Vermont
Strong communication and interpersonal skills
Ability to work effectively as part of a team and adapt to changing priorities
Strong organizational and time management skills
