Job Summary:

Polaris Healthcare and Rehabilitation Center, a compassionate and respected healthcare provider in the greater Milford community, is seeking a full-time Unit Clerk to join our dedicated team. As a Unit Clerk, you will play a vital role in providing exceptional care and support to our residents and their families. Responsibilities:

  • Manage a busy nursing station by answering the telephone and responding to resident and/or visitor questions

  • Coordinate follow-up doctor appointments that may need to occur outside of the facility for residents

  • Maintain resident charts, assist unit manager with all clerical duties, telephone interaction, data entry, and filing

  • Work directly on the nursing floor with resident and family interaction

  • Manage the filing/scanning of information into the medical chart for each resident

  • Assist the unit manager with all clerical duties

  • Organize and maintain paperwork in the resident medical chart

Qualifications:

  • CNA preferred

  • Must possess excellent communication, interpersonal, and organizational skills

  • Unit Clerk experience preferred

  • Experience in long-term care preferred

  • High school diploma or equivalent
