Responsibilities:
Provide direct patient care, including administering medications, performing treatments, and documenting patient records
Develop and implement personalized care plans in collaboration with other healthcare professionals
Monitor patient status and report any changes or concerns to the healthcare team
Collaborate with other departments, such as physical therapy and occupational therapy, to ensure comprehensive care
Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
Maintain accurate and detailed records of patient care and interactions
Educate patients and families on disease management, medication usage, and other aspects of care
