Responsibilities:
- Conduct skilled nursing visits to patients in their homes throughout the Grand Junction area
- Perform assessments, wound care, medication administration, and other clinical nursing duties
- Monitor and document patient health status, progress, and changes in condition
- Communicate effectively with RNs, physicians, and care team members to coordinate patient care plans
- Educate patients and family members on care techniques, medications, and health management
- Maintain accurate and timely clinical documentation in compliance with home health regulations
- Provide compassionate, patient-centered care while promoting independence and dignity
- Active LPN license in the state of Colorado (required)
- Prior home health, visiting nurse, or field nursing experience preferred
- Strong clinical assessment and wound care skills
- Reliable transportation, valid driver's license, and auto insurance
- Proficiency with electronic health records (EHR) and clinical documentation
- Excellent communication, time management, and organizational skills
- Compassionate, self-motivated, and able to work independently in a field-based setting
