We are seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our team at Tucker House Nursing and Rehabilitation, located in Philadelphia, Pennsylvania. About the Role:
As a CNA at Tucker House, you will be responsible for providing high-quality care to our residents, including:
Assisting with personal care activities, such as bathing, dressing, and grooming
Supporting resident mobility and transferring residents safely
Assisting with mealtime and hydration
Monitoring and reporting changes in resident condition
Collaborating with the interdisciplinary care team to develop and implement individualized care plans
Providing emotional support and companionship to residents and their families
Responsibilities:
Provide direct patient care to residents, including vital signs, wound care, and medication assistance
Document resident care and progress accurately and timely
Participate in resident care conferences and contribute to the development of individualized care plans
Maintain a safe and clean environment for residents, staff, and visitors
Collaborate with other departments, such as rehab and activities, to ensure comprehensive care
Provide emotional support and companionship to residents and their families

Requirements:
Current certification as a Certified Nursing Assistant (CNA) in the state of Pennsylvania
High school diploma or equivalent required
Ability to work varying shifts, including days, nights, weekends, and holidays
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
