Responsibilities:
Provide personal care and assistance with daily living activities, such as bathing, dressing, and grooming
Assist with mobility and transfer patients, as needed
Administer medication and maintain accurate records
Monitor and report any changes in patient condition to the Nurse, Physician, and family members
Participate in care planning and development of individualized care plans
Maintain a clean, safe, and comfortable environment for patients
Collaborate with the healthcare team to ensure continuity of care

Requirements:
High school diploma or equivalent required
Completion of a state-approved Home Health Aide training program
Current certification as a Home Health Aide
Ability to lift, transfer, and reposition patients as needed
Strong communication and interpersonal skills
Ability to work independently and as part of a team
