As a Home Health Aide or Personal Care Aide, you will be responsible for providing assistance with daily living activities, such as bathing, dressing, and grooming, as well as light housekeeping, meal preparation, and medication management. You will work closely with our team of healthcare professionals to develop and implement personalized care plans that meet the unique needs of our patients.
Responsibilities:
Provide daily living assistance to patients, including bathing, dressing, and grooming
Assist with light housekeeping, meal preparation, and medication management
Develop and implement personalized care plans in collaboration with our healthcare team
Maintain accurate and detailed records of patient care and services provided
Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination
Adhere to high standards of patient care, safety, and infection control
Requirements:
Home Health Aide or Personal Care Aide certification or training
High school diploma or equivalent required
Ability to lift, bend, and move patients as needed
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Reliable transportation and a valid driver's license ( wenn available)
