What You'll Do

• Evaluate residents and develop individualized physical therapy plans of care

• Provide skilled PT services focused on mobility, strength, balance, endurance, safety, and independence

• Establish goals and monitor resident progress

• Help residents achieve their highest possible level of function

• Complete accurate and timely documentation using Net Health

• Utilize PointClickCare (PCC) as appropriate

• Collaborate with nursing, therapy, physicians, families, and the interdisciplinary team

• Provide supervision and clinical guidance to PTAs as appropriate

• Participate in discharge planning and promote safe transitions

• Maintain compliance with applicable state, federal, facility, and professional standards

What We're Looking For

• Graduate of an accredited Physical Therapy program

• Current New York Physical Therapist license or eligibility for NY licensure

• SNF/LTC experience preferred, but we welcome qualified candidates from a variety of backgrounds

• Strong communication and organizational skills

• Positive, team-oriented approach

• Passion for helping residents achieve their goals
