We are seeking a highly skilled and dedicated Certified Nursing Assistant to join our team at Bronte Health and Rehab Center, a Skilled Nursing Facility. Responsibilities:

• Provide certified nursing assistant services to assigned residents in accordance with care plans, facility policies and procedures, and at the direction of supervisor(s)
• Assist nurse(s) and other staff as needed, and perform duties as assigned
• Assist residents with or perform activities of daily living for resident in accordance with care plans and established policies and procedures
• Assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
• Coordinate dining room services at assigned mealtimes, including set-up and clean-up, meal tray delivery, feeding assistance, and documentation of meal intake
• Deliver nutritional supplements to residents at assigned times and provide assistance as necessary to ensure intake, documenting intake accordingly
• Assist with weighing residents according to facility policy, and record weight in designated locations, reporting weight changes to nurse and supervisor
• Assist with tracking the condition of the resident's skin, reporting any presence of pressure areas, skin breakdown, or skin tears to nurse and supervisor
• Assist nursing staff in carrying out toileting program activities
• Complete flow sheets daily to indicate that the specified task was done, documenting the progress of each resident as directed by supervisor
• Keep nurse and supervisor informed of factors that interfere with being able to perform the work as assigned (i.e. resident refusal, reassigned to nursing unit)
• Report changes in a resident's condition to the resident's nurse and supervisor

Requirements:

• Certified Nursing Assistant in good standing with the state
• High school diploma or equivalent
• Current CPR/BLS certification
• Ability to work effectively as part of a team, and to communicate effectively with residents, families, and staff
• Ability to lift, move, and position residents, and to assist with transfers and other physical activities
