Nurse Practitioner or Physicians Assistant
Job Description
The Advanced Care Practitioner will primarily encompass clinical work, post-operative follow-up care, and may also include participation in physician-led research projects. The ideal candidate will be a “self-starter” with prior pain management, orthopedic or neurosurgery and must possess a passion for excellence.
Essential Function / Responsibilities

· Assesses patient health status, including state of wellness and compliance with care plans
· Meets with new patients; obtain and update patient history and monitor ongoing treatment
· Triages patient calls and evaluates patient concerns, providing guidance as needed
· Provide acute and chronic pain care, including taking histories, monitoring therapies, giving injections and providing post-operative care
· Documents patient information and care provided in the patient record
· Educate patients and families as appropriate
· Provides continuity of care
· Maintains privacy, confidentiality, and security of patient, client, staff, and organizational data
