Behavioral Case Manager

Works effectively with the facility leadership team to ensure success of the facility by completing the following:

• Interview clients, family members or caregivers may also be involved in the interview process

• Reviews records related to medical treatment or previous health services, and develops a plan of care with the patient and family

• Is careful to understand the needs of the clientele by scrutinizing their background history, understanding their current needs, and arranging for their well-being.

• Communicate regularly with residents and their families about Plans of Care, OT and other treatment protocols

• Answers residents' questions about their care, treatment plans, illness progression and all issues, so they feel safe and secure in our care

• Devise procedures and protocols for patient admission and filtration so each resident receives consistent, attentive care through the duration of their stay with us

• Monitor and adjust patient statuses based on changing needs and conditions

• Coordinate referrals both to and from our facility

• Address any patient concerns with appropriate in-house departments

• Oversees all intake and discharge activities

• Ensures that the clients receive quality care and that the organization complies with federal, state, and local laws

• Proficiently audits documentation to ensure compliance to company policy, state regulations, Joint Commission Standards, Medical Needs Criteria.

• Ensure quality service delivery at all levels

• Teach client classes as assigned

• Conduct quality assurance audits as assigned, and any other duties assigned by supervisor.
