About the Role:
D Square Healthcare Services Inc is looking for a compassionate and dedicated Direct Support Professional to join our team in Shakopee, MN. In this rewarding role, you'll make a meaningful difference in the lives of individuals with disabilities by providing personalized care and support. Responsibilities:
- Provide direct care and daily living support to individuals with intellectual or developmental disabilities
- Assist clients with personal hygiene, grooming, meal preparation, and household tasks
- Implement individualized support plans (ISPs) and document progress accurately
- Transport and accompany clients to medical appointments, community activities, and errands
- Administer medications and monitor health conditions per care plan guidelines
- Foster independence, dignity, and community inclusion for each individual served
- Communicate effectively with supervisors, families, and healthcare team members
Requirements:
- High school diploma or GED equivalent required
- Previous experience as a DSP, caregiver, CNA, or in a related healthcare/human services role preferred
- Valid driver's license and reliable transportation
- Ability to pass a background check as required by Minnesota DHS
- CPR/First Aid certification (or willingness to obtain upon hire)
- Strong communication, patience, and problem-solving skills
- Genuine compassion and commitment to person-centered care
